    I received an invoice but I have insurance. Can you bill by insurance?
    Yes, if the information received is within the filing deadline set forth by the carrier. Please contact us at 915-755-8478 with the required information.
     
    Can I fax my insurance information?
    Yes. Please make a copy of your card (front and back) and fax it along with your invoice to 915-755-7331. Please include a return fax number and a daytime phone number. You will receive a return fax confirming the information was received and adequate to submit the claim to your carrier.
     
    I gave my insurance information to my healthcare provider at the time of my visit. Why was this not forwarded with my lab work?
    We rely on what the referring physician’s office provides us. Sometimes pertinent information is left blank or not attached and therefore we must send you an invoice.
     
    Why didn’t my insurance pay?
    There could be numerous reasons why your insurance carrier did not pay the claim. Refer to your Explanation of Benefits (EOB) for a complete description. Please call us at 915-755-8478 and one of our Patient Billing Account Representatives will help you to understand the Explanation of Benefits.
     
    Will my insurance carrier cover the tests my healthcare provider ordered?
    It depends on your particular plan whether or not the service will be covered. Please refer to your plan book for additional information. It also depends on why you had the test. Example: A pregnancy code would be denied if you do not have maternity coverage included in your plan.
     
    My insurance carrier applied all of the charges to my deductible. Can I make payments?
    Yes. Please call us at 915-755-8478 and one of our caring Patient Account Billing Representatives will set you up on a payment plan.
     
    I have secondary insurance. How do I file a claim?
    Mail a copy of the invoice and a copy of the Explanation of Benefits from your primary carrier to the claims address listed on your secondary insurance card. GYN PATH does not automatically file secondary insurance claims.
     
    What methods of payment does GYN PATH accept?
    Personal Check, Money Order, Visa, MasterCard, American Express and Discover.
     
    Why do I receive multiple invoices for tests performed on the same day?
    All lab work is issued a specific identifiable account number and each test is treated as a new account. Therefore, you will receive an invoice that correlates with each account number for each lab test that was performed.
